Active Re-Entry is a community-based program that assists individuals with disabilities to achieve or maintain self-sufficient and productive lives in their own communities and home.

Carbon, Emery, Grand:

Southeast Utah Health Department - Health Department for Carbon, Emery, and Grand Counties.

Four Corners Behavioral Health - Four Corners Behavioral Health is a 501.C3 located in Central and Southeastern Utah providing Mental Health and Substance Use services.

Southeastern Utah Association of Local Governments – Covering Carbon, Emery, Grand, and San Juan Counties. SEUALG helps with rent assistance, aging, H.E.A.T, weatherization, and more.

Moab Valley Multicultural Center – The Moab Valley Multicultural Center provides bilingual crisis intervention and advocacy services for anyone with socioeconomic, cultural, or language barriers with particular emphasis on immigrant, minority, indigenous, and vulnerable populations of all ages.

Moab Free Health Clinic – The Moab Free Health Clinic is a nonprofit organization established in 2008 to ensure that all members of the Moab community have access to medical care and health resources regardless of race, religion, age, gender, health care coverage, or financial need.

Uintah Basin:

Tri-County Health Department - Health Department covering Uintah, Duchesne, and Daggett counties.

Uintah Basin Association of Governments - Covering Uintah, Duchesne, and Daggett counties. UBAG helps with rent assistance, aging, H.E.A.T, weatherization, and more.

Basin Transit Association – Public Transportation Service offering bus routes between Vernal, Roosevelt, and Duchesne.

San Juan:

San Juan Public Health – Promoting health and preventing illness and injury in San Juan County.

Utah Navajo Health System - A not-for-profit Community Health Center providing medical, dental, and behavioral health care.

State:

Ability 1st Utah – Center for Independent Living, providing services in Utah, Wasatch, Juab, and Sanpete Counties.

Common Ground Outdoor Adventures - life is full of adventures! People and life experiences are our foundation and that is why we are called Common Ground Adventures.

Options for Independence - Center for Independent Living, providing services in Box Elder, Cache, and Rich Counties. They have a satellite office in Brigham City.

RRCI - Center for Independent Living, providing services in Millard, Sevier, Beaver, Piute, Wayne, Iron, Garfield, Kane, and Washington Counties. Satellite offices in Cedar City, Delta, and Richfield.

Roads to Independence - Center for Independent Living, providing services in Morgan, Weber, and Davis Counties.

Splore - Outdoor Adventures & Recreation for People with Disabilities and Special Needs.

Utah Independent Living Center (UILC) - Center for Independent Living, providing services in Salt Lake, Summit, and Tooele Counties. Satellite Office in Tooele.

Utah Best Friends - Best Friends Animal Society in Utah is working collaboratively with animal rescue groups, city shelters, and passionate individuals who are all dedicated to the mission of making Utah a no-kill state.

U.S.I.L.C. - Utah Statewide Independent Living Council

Utah State Office of Retirement (USOR) - Mission is to assist eligible individuals with disabilities in obtaining employment and increasing their independence.

Division of Services for the Blind and Visually Impaired (DSBVI) - DSBVI Helps blind and visually impaired Utahns achieve their goals, whether through education, employment or living independently in their home.

Utah Division of Services for People with Disabilities (DSPD) - The Division of Services for People with Disabilities (DSPD) promotes opportunities and provides supports for people with disabilities to lead self-determined lives by overseeing home and community-based services for more than 6,000 people who have disabilities. Support includes community living, day services, and supported employment services.

Utah Pride Center - Utah Pride unites, empowers and celebrates Utah’s diverse LGBTQ+ community by providing a safe and welcoming space for education, partnerships, services and events which advance our collective health, wellness and success.

Utah State Library for the Blind and Disabled - The Program for the Blind and Disabled provides the kinds of materials found in public libraries in formats accessible to the blind and disabled. Books and magazines are available in braille, in large print, and on audio digital books.

VA Hospital - Through the VA Salt Lake City Healthcare System, our expert health care teams focus on your needs and the needs of other Veterans, your families, and caregivers.

National:

ADA – Information and technical assistance on the Americans with Disabilities Act.

APRIL - APRIL provides leadership and resources on rural independent living

ILRU - ILRU, founded in 1977, has a long history of providing research, education, and consultation in the areas of independent living, home, and community-based services, and the Americans with Disabilities Act.

NCIL - NCIL advances independent living and the rights of people with disabilities. NCIL envisions a world in which people with disabilities are valued equally and participate fully.
