Programs

Low Vision Program

Providing education in adaptive techniques, devices, peer support groups, peer counseling orientation, mobility training, information on resources and benefits available to those with low vision in addition to social and recreational activities.

Assistive Technology Program

Funding is made possible through the Department of Workforce Services (DWS), the Utah Assistive Technology Foundation (UTAF), and other funding sources to purchase assistive technology equipment for individuals with disabilities. An intake and assessment is conducted to identify the equipment required to increase the individuals independence at home and in the community. The Assistive Technology program is provided to individuals in need, who meet the financial eligibility criteria.

Youth Transition Program

Brings together youth ranging from 8 – 24 years of age. This community-based program helps to build self-confidence, provides independent living skills training, advocacy, and increases communication and socialization skills. Enabling the youth to transition into adulthood.

Music & Memory Program

MUSIC & MEMORY℠ brings personalized music into the lives of elderly or disabled individuals through digital music technology, vastly improving their quality of life. It has been scientifically proven that these musical favorites tap deep memories, assist with pain management, and can bring individuals back to life, enabling them to feel like themselves again, converse, socialize and stay present. Active Re-Entry is certified as a MUSIC & MEMORY℠ provider and can provide personalized playlists for individuals living in the community. Our playlist includes over 5,000 songs and we can obtain specific songs as requested. The iPods, headphones, and setup are free of charge.

PERKIE Travels Program

Free daily transportation service for cancer patients from Carbon & Emery Counties traveling to Utah County for radiation treatments. Transportation is provided to the following locations: Utah Valley Medical Center, Revere Health in Provo, and Timpanogos Regional in Orem.

Community Integration Program

Consumers 25 years of age and older meet as a group to develop and improve their independent living skills, self-confidence, advocacy abilities, increase communication, and socialization skills. This group also enjoys participating in recreational activities and attending community events.

Equipment Loan Bank Program

Active Re-Entry’s Equipment Loan Bank Program consists of loaning a wide range of assistive technology devices to individuals on a temporary basis. The loan bank is made up of equipment that has been donated to the Center or purchased through grants. There is no fee. The loan bank items are loaned out on a case-by-case basis, depending on what items are available at the time. Items from the Loan Bank are often used by individuals as a temporary solution while they are healing from surgery or while they are waiting to receive funding to purchase equipment that is needed permanently. Some examples of the items available are wheelchairs, walkers, canes, knee scooters, shower benches, transfer benches, grab bars, toilet seat risers, and a limited number of low vision products.

Friendly Visitor Program

The Friendly Visitor Program is designed to offer friendship and support to isolated or homebound individuals through regular home visits or phone calls. Other services offered are grocery shopping, bill paying and assisting with other unmet needs.

Transportation

Active Re-Entry provides door-to-door transportation to our consumers for certain Active Re-Entry activities if provided with a 24-hour notice.

Interpreter Referral

Matching consumers with a qualified interpreter to facilitate effective communication.

Support Groups

Grief Support Group

Monthly meetings are provided to provide support to those experiencing grief from the loss of a loved one. Meetings are held on the 2nd Wednesday of the month.

Caregivers Support Group

Monthly meetings are offered to assist and support family members or others who are caring for individuals with disabilities. Meetings are held on the 4th Wednesday of the month.

Active Access Support Group

Monthly meetings to unite individuals who use Assistive Technology for mobility within the community. Includes training, education, and recreational opportunities. Meetings are held on the 2nd Thursday of the month.

Low Vision Support Group

Monthly meetings are held to provide education and support to individuals who are losing or have lost their vision. Door to Door transportation is available for this group on an as-needed basis. Meetings are held on the 3rd Thursday of the month.
